Rare item indeed--I cannot find one like it anywhere on the Internet or eBay!  I did find an old advertisement for it, dated 1952, in the Pittsburgh Post-Gazette.  This 4-in-1 wooden alley game measures 52" long, 10" wide, and 18" tall.  It has character for sure, but it is not without issues/wear:
  • One pin is missing.
  • The pins and ball do not "appear" to be original to the game.
  • The game board is covered with red scribble that looks like crayon.  It may come off somehow, not sure.
  • There are scuffs all over as typical with something so old.
  • There is damage to the wood piece on the top back.
  • There are other nicks/wear throughout.
  • The front legs underneath need to be repaired (pointed to in 2 photos).
The only things that are included are what is pictured.
