The advice we had from xbox support is to redeem the code from the xbox website rather than from the console. If this does not work you then have to do a hard reset > remove and re-add the account. 

This is how to do it:

1. Press the Xbox button  to open the guide.

2. Select Settings. 

3. Select All Settings. 

4. Select System. 

5. Select Console info

6. Select Reset console.  The please make sure to select Reset and keep my games & apps.
