PROGRAMMING INFORMATION:

Please note that the audiogram must be legible, current (within the last 6 months), include patients full name and signed by a licensed practitioner.

We must have a copy of your valid audiogram on record in order to provide programming to any hearing aid.    

Note: Due to Florida state law we are unable to ship hearing aids to Florida residents.
