Historically accurate, faithful reproduction of a photo ID which was awarded along with German Mother's Cross of Honor. It comes blank. Example in the picture is filled out on a typewriter and a photo affixed. It has 4 pages and comes with accurate national symbols. Medal in the pictures is not included.

It can come to you customized with data of the happy mother and her picture (all filled out) for additional $9 - Please choose expedited shipping if you go for that option and you will need to provide me with the following data:
- name of the mother
- her maiden name
- place and date of birth
- award date
- grade of the awarded medal (1st, 2nd, 3rd)
- send me the passport photo of the mother - I will print it on photo paper and affix with stapler
She will still need to manually sign it under the photography.
