The manufacturer determines the belt size

 by measuring from the belt fold 

(where the belt fastens to the buckle) 

to the middle belt hole. 

To choose the proper belt size

 you should add at least 2 in/5.1cm

 to your waist size. 

For example:

if your waist size is M34/86, 

you should choose a belt size 

of at least M36/91.
