Note:  There are dozens of different HID ballasts made:  Please check the image to see if the connections match your ballast.  No Returns! 

Note:  There can be many causes of headlight failures.  Before replacing the ballasts, swap the headlight bulbs to the opposite sides and see if the problem is the bulb or ballast.  If the bulb still does not light after you swap it, then the problem is with the bulb and not the ballast.
