This is a footrest assembly from a Koken Premier barber chair.  I have been told it is also for a Koken President barber chair.  It has the two original mounting bolts with it.  As you can see by the pictures, the leather covering need some attention.  There is one screw missing for mounting the calf pad and I am not sure if the other screws are original.  Overall in very good condition.  These are very hard to find.  Shipping will be $36.00 to the lower 48 states.
