This is an original IBM 008216 Multi-Port USB Hub with PS2 (Mouse and keyboard), Parallel (IEEE 1284) Printer and Serial (RS-232) Ports. It includes the Main Hub, USB Cable, AC Power Adapter and instruction manual.  It does not include the drivers which you should be able to find online (I have not checked). Cosmetically, I would rate this a 9 out of 10 with only minimal signs of use.  Feel free to ask any questions.
