MINT-Pack fresh and flawless.

NM-Near mint. Card should look pack fresh or real close to it.

EX/NM-Somewhere in between excellent and near mint.

EX-Excellent. Card shows signs of play that could be in the form of edge wear, surface wear, and/or minor amounts of shuffling damage, but it is still an ascetically pleasing card, looks almost NM in a sleeve a lot of times.

G/EX-Somewhere inbetween good and excellent.

G-Good. Showing signs of wear in the same form as in excellent, but to much more extensive amounts to the point where the wear is easily recognizable in a sleeve and the card is no longer ascetically pleasing.

HP-Heavily Played. Although still a discernable card and tournament playable, a heavily played card can be one of a number of things: worn past being in good condition, has a tear or a crease, been water damaged. If there is not a note about water damage, tears, or creases though, you can assume it's just a very worn card. I'll make notes about the condition in the title or the description if any of these three things have occured.
