This is from the the recently deceased David Kurek Estate. David was life long collector who dedicated himself to attending top collector shows (Chiller Theatre, Hollywood Collectible Show, FrightFest, Glamourcon, etc,) throughout the country for decades. He was a consummate buyer of unusual and eclectic items many of them signed/personalized. Dave also had amassed a sizeable collection of photos of himself with celebrities.
