A high performance active crossover filter by  KMTech Design.

This is a 2-channel (stereo)board. Each channel has a 3-way frequency split, but the subwoofer output is mixed (mixed by summing, not simply hardwired together) and combined into a single output, giving five outputs in total. The default crossover frequencies are 67 Hz and 1.7 kHz.

You could also use this board as a standard stereo 3-way by bypassing the LF mixer (take the bass outputs from the LH side of R17 and R18).

Let us know what crossover frequencies you need  and we will get as close as possible by changing resistor values.
For your information, the formula for  calculating the filtering resistor values is:
R = 1 / (8.89 x C x f)
Where R = resistor value in Ohms
f = Desired crossover frequency in Hertz
C = 0.000000039 for the lower crossover frequency.
C = 0.0000000033 for the higher crossover frequency.
