This item is a Heath Zenith motion 150 degree energy save lighting control, part number SL-6115-WH.  It is new in sealed package.  It came from a lighting store that closed.

The single pole motion sensor wall switch automatically controls indoor lighting. In AUTO mode, the control automatically turns the light on when motion is detected. The motion sensor keeps lights on for a set period of time after motion is last detected.Installation is as easy as replacing a  wall switch
