This is the vinyl maxi-single "Personal Jesus", from the album "Violator", by Depeche Mode. It was released on Sire Records, catalog number 0-21328. This vinyl single is an original first pressing and was released in 1989. It includes the tracks:

The record itself is in Excellent (EX) condition with just about no visible marks and absolutely no scratches. The cover is also in better than Very Good (VG+) condition with minor wear and is still wrapped in its original shrink wrap. Great buy!
