This is a more unusual mold style to find the CD 121 AM. TEL & TEL CO. toll insulator. This is the Denver mold with a more unusual shaped upper lip and dome.

Perhaps 10-15 literal scattered pin pricks, a few dome top fleabites l, and I'd not rule out some other needlepoint bites and pricks.
