VG + hardcover in a VG dustjacket  .. The booklet , also a first printing is also VG + 

Both the book and the booklet have gift indcriptions to a profesor , who , I would assume was a teacher of Kosinski's .

The booklet is signed with a different pen, presumably at a different time .

 The Notes book is usually sold separately 

There are no notes , underlining or stains in either book ..The dustjacket has no tears , fraying or stains 

There is also a several pages of typing  ( which looks original, not copied ) entitled " What the Early Advance Readers said About The Painted Bird ), dated 0ctober 14 , 1965 ..  Maybe this is a precursor of a printed ad ...
