US Armed Forces typically carry two identical oval tags containing:

US Air Force Format 1:

  1. Surname, first name, middle initial
  2. Social Security number, followed by "AF" indicating branch of service
  3. Blood Group
  4. "blank"
  5. Religion (or "NO RELIGIOUS PREF" if none)

U.S. Air Force Format 2:

  1. Surname
  2. First name and middle initial
  3. Social Security number, followed by "AF" indicating branch of service
  4. Blood Group
  5. Religion (or "NO RELIGIOUS PREF" if none)

US Marine Corps:

  1. Surname
  2. First and middle initials, blood group
  3. Social Security number with no dashes and no spaces
  4. Branch ("USMC"), Gas mask size (S-small, M-medium, L-large)
  5. Religious preference (or "NO PREFERENCE"), or medical allergy if red medical tag

US Navy:

  1. Surname, first name, middle initial
  2. Social Security number with no dashes or spaces followed immediately by "USN", space, blood group
  3. "blank"
  4. Religion (or "NORELPREF" if none)

US Army:

  1. Surname
  2. First name, middle initial
  3. Social Security number
  4. Blood type
  5. Religion

US Coast Guard:

  1. Surname, first name, middle initial
  2. Social Security number, no dashes or spaces, followed immediately by branch (i.e., 123456789 USCG)
  3. Blood group
