Double Panthers Stamp. Not sure of the disc but on the front it states:

"Designed and Produced Exclusively for Wham-O by Innova Champion Discs, INC. Frisbee is a brand name and registered trademark of WHAM-0 San Gabriel, CALIF 1987"
