This is a Mortal Kombat arcade stick for the Xbox 360.  It comes with the original box, CIB Mortal Kombat game disc and USB cable.  The controller itself is in perfect condition, but the outer box has a few tears.

I've also tested this controller via its USB cable on a PC and an Raspberry Pi and both worked perfectly.
