This is a chart which is designed to help a person young and old to organise and check that they have completed the main aspects of daily personal care. It is laminated and A4 in size. There is the option to have it with either tokens to attach or with a dry wipe pen to write on the chart. This can also be used as a motivational reward chart by allowing the person using it to gain a reward after so many correct routines are completed they may gain a reward. This chart clearly shows the person how they are doing and what they may have missed. This chart is designed to help the person become more independent and take more control of their lives, also allowing an opportunity for the carer to respond to aspects that may be missed on a regular basis.

 

This chart is great for use at home, care setting or hospital. Think about ordering two packs and use them at other places the person needs to go regularly as consistency in identifying the triggers and behaviour is very important.
