When new this came with a 12"x12" thick supplement card which was very lightly adhered to the rear cover below the clear outer wrap - it fits neatly inside the book when the cover is removed. The first line on the top of the card is: "Save this supplement for Audio and Chapter information"

The chapter listing on the inside of the "DELUXE COFFEE TABLE BOOK" are incorrect - they are correctly listed under the "revised Chapters" listing on the card supplement. The Completely Cuckoo Original Documentary is listed as the "Original Documentary" with the "Approximate Supplement Running Time" listed as 60 minutes. The Completely Cuckoo running time is 87 minutes - with the trailers included the total Approximate Supplement Running Time is just over 90 minutes.
