new 3dfx chip for Voodoo 4 / voodoo 5 VSA 100 with label 355-0026-220, but  the item may have some signs of cosmetic wear because of ages, storing, but is fully operational and functions as intended. That is information what i find but before buying please search by yourself if this number 355-0026-220 is really for voodoo 4 and voodoo 5, i am not expert in this topic, this was my fast searching.
