This is Veerabhadhra, a form of Siva. 

It is from North Karnataka in South India.

Siva took this form to punish Daksha. 

It is about 7 inches tall (17.5 cm).

Difficult to get one with the back plate. The backplates are detachable and invariably they get lost.
