The fourth Festival medal is the sixth issue connected with music. Two were issued previously in connection with the Harp competition in Jerusalem in 5719 and 5722 (1959, 1962). Three were issued in honor of the annual festival in 5721, 5722 and 5723 (1961, 1962 and 1963).

On the obverse of the medal appears the text "Israel Festival 5724" in raised letters, the Festival emblem, and the English inscription "The Israel Festival 1964".

The reverse shows four incuse rectangles of different sizes. The top portrays the State emblem. The bottom, a theatrical mask. On the left, a four string harp. Inside the largest rectangle is the inscription in English: "Presented by the Prime Minister's Office".
