This is a handcarved Hawaiian Hei Matau fish hook necklace, made of carved Buffalo bone (bos Taurus). It is lashed to an adjustable black hemp cord. The longest length of the cord is 27 inches. The hook itself is 1-⅜  inches tall and ⅞  inch wide.

Traditionally, a high quality fish hook was a very valuable possession. Over time, wearing a fish hook around your neck became a symbol of prosperity and abundance. It also came to symbolize good luck and safety while traveling over water.
