This is a barely used Gemplus GCR432 USB Smart Card Reader branded with the Fusion Visa Logo.  The card reader was tested under Linux.  It turns on and appears to sense the presence of a smart card.  It does not come with a software CD nor instructions.  It is originally compatible with Windows and also has drivers for Linux.
