This transformer is a model L01- 6014. The primary voltage is 115 VAC and the secondary voltage is 18 VAC @ 1.6 VA.

L x W x H = 44.7 x 44.7 x 19.5 mm. You can attach it to a circuit board with a M4 or self-tapping screw.
