Older Fostoria USA industrial spring-loaded swivel articulating arm magnifying lamp model  MAG-S45-CO. 

Table/bench mounted with two bolts and screw clamps included.  A tab on other side of base has a hole for more permanent mounting.  5" Lens, 8" fluorescent ring bulb (included).  Rocker switch and single extra electrical socket in the base. 

7.5' black power cord has electrical tape added 2' from the plug most likely to insulate bare spots.  The ground prong has been removed.  Both issues pointed out in the last picture.

This dark chocolate brown fixture does have scuffs and paint specks.   The lens is clear and powerful.  Springs strong with no rust.  Joint tightness adjustable to hold whatever position you choose.
