This is a Panasonic AJ-PCD20P 5 slot P2 Card Reader with FW800 (1394b), USB 2.0 ports. The original Panasonic Power Supply is included too.

The Panasonic AJ-PCD20P is a P2 drive which acts as a conduit between P2 cards and a computer. Specifically for use with non-linear editing systems, this drive can ingest up to 5 P2 cards at one time over FireWire or USB 2.0. When connected to either a MAC or PC the P2 cards will appear as one drive, making it extremely easy to browse through video footage.
