This is used it is a 5 blade fan. The fan blades are plastic. I did test the fan using 12 volts it works as intended.  Both speeds worked with the switch. I used the base of the  fan as ground and put plus to the red wires. The fan needs additional clean up, but it is just dust from years of non use.
