Disc: Nearly perfect. Handled with great care.

Insert/Inlay/Booklet/Sleeve/Digipak/Jewel case: Near perfect, minimal wear from handling.

Very Good Plus (VG+) 

Disc: There are a few light scuffs and scratches, but it doesn't affect playback.

Insert/Inlay/Booklet/Sleeve/Digipak/Jewel case: There is light wear, marks, and indentations, and there is a possibility of a cut-out hole (or similar).

Very Good (VG) 

Disc: There are quite a few light scuffs and scratches, or several more pronounced scratches. It doesn't affect playback.

Insert/Inlay/Booklet/Sleeve/Digipak/Jewel case: More wear, marks, indentations than a VG+. May have slight fading, a small tear/rip, or some writing.

Good (G), Good Plus (G+) 

Disc: There are a lot of scuffs/scratches. However it will still play through without problems. This has not been handled with much care at all. 

Insert/Inlay/Booklet/Sleeve/Digipak/Jewel case: Shows considerable wear, markings, more noticeable indentations, fading, and writing compared to VG - possibly a more pronounced tear or rip.
