The card is is good condition and has wear with light creases, warping and wear around the edges. The top left corner side is torn and missing some pieces.  The bar code on the back side is marked through with a price written above.

The plastic is firmly affixed to the card with some slight pulling away on the top left corner.  There are also some light creases in the plastic.  

Comes from a non-smoking home.
