You are looking at a Fender Vibro Champ Amp. AA764 Design and Circuits. Patented 1964. Production 97 on tube chart. Made in Fullerton California USA. There are two numbers on the chassis. The number inside that is a stamped ink number is...T184866, and the stamped number on the outside back of the chassis is...A15679. This leads me to believe the amp may be a 1966 or 1967 production year? Tubes are...JJ Power Tubes: 5Y3GT, 6V6GT, and(2) 12AX7. Two 1/4 inch input jacks, Volume, treble, bass, speed, and intensity dials. 1 amp slow-blow fuse, .5 amp, 117 volts, 60 cycles. Original 2 prong electrical cord. Also comes with padded Tuki snug fit cover. 

I have owned and enjoyed playing through this amp over 15 years. Good cosmetic tolex, superficial tarnish from its age, all dials work correctly, no hums or hisses. I believe this amp to be all original and correct as the day it left the production plant.
