We do not have much knowledge of this item and it is brand new and will need to be wired into wherever you decide to use it.  The fan is a classic built to last as it was built in Ohio.  I believe these were made for a Hi-Lo but honestly you can wire into wherever you wish or attach a cord to it.

The small yellow envelope has some small accessories / hardware and was in the box when we acquired it.  Due to my lack of knowledge, I cannot say that all hardware is present, but it looks to all be there and things like wire caps, wingnuts etc can be replaced with very little cost.

The original box is intact and we have priced this well below what others have sold for. 

Shipping info: We do not profit from shipping rates and when shipping our clocks and heavier items we use ebays “cost by location”.  These rates are based off of weight and location with packing material included. We also combine shipping whenever possible.
