Cover is in very solid shape for its age. Some light splitting in the center of the top edge. Some bits of edge wear here and there. No ring wear.

Record is in fantastic shape! Very clean just very light scratching! maybe VG-VG+

I'm not quite sure what version this is. Looking at Discogs lists of versions it most closely matches with black label mono Hollywood version but the label isn't quite right. The placement of words don't quite match.
And it isn't the MGM one either.

Regardless, it is in nice shape!
