Vintage NHL Stars hockey game made in Canada by Eagle Toys.
Appears to be complete but not sure. The teams are the Chicago Black Hawks and the Toronto Maple Leafs.
The levers all work and overall the game looks very good.
The cardboard scoreboard has one plastic clamp broken but holds on with one.
