Is untested.  Included everything as shown, very complete with the different wiring harness and connections.  The corner of the pcb that slides into the slot to help hold is broke off as seen in photo.  There are no runs or solder pads on this area does not effect conductivity in any way and the piece of wafer is included.  Hope that helps.  If you have any questions just ask.
