This is the DAA module used on older PC Socket 370 Motherboards, like the PC-Chips M748MR, M741LMR, P6SE-ML and P6SET-ML. Please see the pictures for an extract from the motherboard manual for pinout information.

The information in the motherboard manual reads as follows:

"The fax/modem DAA module plugs directly into the mainboard adjacent to an expansion slot in the system chassis. When you remove the blanking plate from the system chassis, you can access the LINE and TEL RJ111 connectors on the metal edge of the Fax/modem DAA module."
