In good, pre-owned condition.  Booklet looks barely used, but there is one page I noticed with writing (see photo).  

There is one loose page, section II page 19, that must have been an addendum because the back of section II page 18 is section III page 1.

Booklet measures approx. 8 3/8" x 5 1/2".
