Formula 5 200mm 1:4.5 lens, to fit Canon FD mount cameras.  The focus and aperture mechanisms work, and the optics appear scratch and fungus free.  There is a little dust, but generally quite clean.  There is some cosmetic wear to the barrel, with some paint rubbing and scuffing.
