The plastic cover palm rest for the music keyboard, I think it also is supposed to have, is lost. Otherwise, keyboard seems in good shape.

Includes original box, but the exterior slipcase is very beat up, has some major wear and tears. 
The interior cardboard box holding the keyboard, however, is in great shape. 
Includes manual. 

I'm including a PS/2-to-USB adapter as well. I bought the keyboard used, and never got it to work (as it required a direct PS/2 connection. Creative Labs never made updates to let it run on modern hardware). Maybe you'll have the proper setup (Win XP, Win 2k, Linux) and better luck. 
I read on Wikipedia that someone got the DM version  of the keyboard working on modern Windows OS with an Arduino microcontroller  (https://github.com/hotkeymuc/ProdikeysDM_Adaptor), so this may be a possible avenue to explore if you want to run it on a modern machine!
