Clear plastic is dented but intact

Cardboard backing is slightly bent but in OK overall condtion

Errors on tag: Birthday on the box is Dec 15, 1997 and tag printed with 1993.
Oak Brook is misspelled on tag and was printed with “Oakbrook” (all one word).
