A memorable medal dedicated to the offensive operation of the Ukrainian Galician Army - the Chortkiv Atheniyu, held from 07 to 28 June 1919, with the aim of liberating the entire territory of the Western Ukrainian People's Republic. The Chortkiv affair, which took place during the most intense period of the Ukrainian liberation struggles of 1917-1921, testified to the will of the Ukrainians to fight and is for the descendants a model of national dignity and courage.

On the obverse of the medal there is the inscription УКРАЇНА (below), under which the Small State Emblem of Ukraine, to the right and left of which years - 1919 and 2019; in the center Trident is depicted, surrounded by branches of Kalina (from the design of the Order of the Ukrainian Galician Army by Mikhail Brynsky); a circular stylized inscription - A QUESTION OF UKRAINIAN GALITIAN ARMY; to the right of the trident is the logo of the Banknote-Mint of the National Bank of Ukraine.
On the reverse of the medal a dynamic stylized composition is depicted on the motive of Julian Krykivsky's painting "Chortkiv's Athenziv", which was a participant of those events: in the center on a mirrored background - a horse-drawn soldier, in the background silhouettes of two riders; inscriptions: ЧОРТКІВСЬКА ОФЕНЗИВА (right semicircular), below is the historical date of the offensive military operation: 07 - June 28, 1919.
