The book is "The Ego and the Mechanisms of Defence" by Anna Freud, published circa 1963 or 1964. 

There is a blank prescription from D.C. General Hospital inside with a 1964 date, and a blank index card. 

Both have yellowed from aging but the prescription appears to be in very good condition. 

There is a book plate in the front cover that says "Jack O. Brady, M.D." 

There appears to be another book plate beneath that, but I cannot read it. It appears hand written.
