there is a stud post behind the rat's skull, which fits into a traditionally pierced ear. 

the tail is supposed to wrap behind the ear to help secure it in place. 

if you have sort of larger ears it can fit in front of your ear and just follow the contour, but may not stay in place as securely. 

its designed for smallish women's ears.
