This is the Black Mickey Mouse Eachez 3" Vinylmation from the D Tour Series #2 .  There are two versions of this item - this is the more common version.  This comes with the box flattened (if you want the box unflattened, please let me know, although shipping will be slightly higher), and the figurine.  This is in mint, unused condition.
