In general: 

It is the highest shipping rate within the order 1st - then an additional added per item. 

For clothing items: would be highest rate 1st and then $1 per additional item won and shipped/paid together. 

For the larger items - depends on the item size and combined weight as the shipping companies sometimes charges more when the size of the box is over a certain threshold.
