This is a classic prairie-style maxi dress from mid-60's/70's.  It is custom-made, homemade dress, so definitely a one-of-a-kind!  Please look at measurements in details.  It is made of a light cotton blend crepe and fully lined with an acetate  fabric.  Gorgeous smocking at waist and sleeves.   Zipper closure in the back.  No major stains or blemishes I could find.  It is a Vintage item, so there is wear and it is not to be expected to be in "new" condition.  It is a lovely piece of the past.
