Civil War requisition form from the 9th New York Artillery, dated March 8th 1865.

On the Special Requisition form:

It measures 10 1/2" x 7 3/4".

Also included is a CDV which my dad said came with this document, but the name on the back does not match any on the form. I wanted to keep them together and you can do some research to see if there's any connection. The CDV measures 4" x 2 1/2". The name on the back looks like "Jay M Kilmer".

This item is from my dad's collection and I try to make sure all details are included, though I'm not very knowledgeable on all items. So if you see an error please let me know and I'll correct it or remove the item if it turns out not to be what it's listed as.
