4 x 4 in (10cm x 10cm) dye-type (neutral gray) polarizer with adhesive + 1 small test card

Comes with pressure sensitive adhesive backing so it can be attached to glass.

It has a protective film on the front that should be removed upon application.  Blocks UV past 380 nm.
