Comes with everything shown in the picture, including the lens, front and back caps, lens bag, cleaning cloth, and the original box and packaging materials.

There is little sign of use on the lens. Image quality is excellent (as new). No haze or fungus on the optics. There may be small amount of dust due to manufacturing process that does not affect image quality.

This lens was purchased Nov 2017 and used a few times. It is to be sold since I will be switching to full frame bodies.
